You will be conducting an interview to assess family history on a client.  The family history will help you to construct a three generational genogram, that will help you to identify the client’s health risks and allow you to create a plan with your instructor to give appropriate teaching regarding any identified health risks or concerns.

Use the following questions to help construct the genogram.  Your instructor will give you a set of symbols to use within the genogram.  You should assess the individual and their generation, and then go back at least 2 to 3 generations if possible (parents, grandparents and great-grandparents).

              CG  (female) died age 61 of stroke complications, suffered Type 2 diabetes, hypertension
Maternal PC (male) died age 76 of lung cancer,  suffered from depression,
              MC (female) died age 53 of pacreatic/gastric cancer, suffered from anxiety, obesity             
-Who lives in the household (initials, age, gender, occupation) ?

-Where do other family members live?
Parents live 30 mins away in Vernon same town as SG veterinary clinic
Sister EP lives 30 mins away in West Milford
Brother MG lives 30 mins away in Rockaway

-What health-related changes or issues has the individual or family members experienced most recently?
SG has recently undergone outpatient treatment for alcohol dependence and has attempted to quit drinking

-Pregnancies, illnesses, hospitalizations, deaths (cause if known).
Recently hospitalized for depression and intoxication, and has subsequently been diagnosed with medication resistant depression.

-Psychiatric illnesses (depression, anxiety, phobias, other mental illness).
SG suffers from depression and anxiety

-Addictions (alcohol, drugs, gambling, etc.)
SG has and alcohol dependence and has demonstrated symptoms of an addictive personality disorder

-Marriages, separations and divorces.
SG never married,  current relationship with DC for 5+ years

-Education (achievements or issues).
Holds DVM degree from Ross University in St. Kitts

-Most recent changes or issues the individual or family may have experienced?
SG has made steps to grow his business and expand operations

Section II: Cultural/Ethnic information and cultural variables.

-Country of origin of people on genogram (include on genogram itself).
Grandparents are second gen polish

-Ethnic/cultural identification: how does this individual describe their ethnic/cultural identity?
SG doesn’t care to identify or participate in Polish

-How does this play a role in their health care decisions/health promotion activities?
Whole family tend to hide illness and keep to themselves

-Religious affiliation.

Parents are very involved in the Polish National Catholic Church.  Does not appear to play a role in health care decisions

-How are health care decisions approached by the individual/family?  Are they similar or different based on assimilation?

Section III:  Construction of the Genogram

Create the genogram for the individual using the symbol chart given and template. Example is in Chapter  of Jarvis textbook.

Include the individual, parents and grandparents ( 3 generational layers).
Include the number of siblings, ages and health issues.  Place on chart in birth order.
Marriages, separations, divorces.
List each member’s health risks and illnesses.
List year of death if known and cause of death of family members.
List member’s country of origin.
Include a legend of symbols (key of symbols used in lower right- hand corner.

Section III: Health Promotion Plan

-Identify the two most critical health risks for this person/family.
Depression and HTN

-Develop a health promotion plan based on findings from genogram to help client mitigate these two identified health risk factors.

For both health risks, include at least three evidence-based actions that can be taken to mitigate risk.
